How to Get a Library Card

The Alta Vista Public Library primarily serves the residents of the City of Alta Vista and residents of the unincorporated rural areas of Chickasaw County.  Other citizens of Iowa are eligible to receive service at the Alta Vista Public Library through the Open Access Program of the State Library of Iowa.  Alta Vista Public Library also offers interlibrary loan services which give our patrons access to materials from across the state of Iowa and the world.

To Apply for a New Library Card with the Alta Vista Public Library:

                1.  Applicant must provide proof of address in the form of a driver's license or other state issued ID or other piece of                         official documentation (water bill, rental agreement, etc).

                 2.  Applicant must provide current contact information in the the form of a phone number and/or email address.

                 3.  Parents/Legal guardians may apply for a library card for their child by providing the required proof of address                               and contact information and signing the child's registration card agreeing that they as parent/legal guardian                                  assume financial responsibility for material checked out on the child's library card.

Visitors to a local household may use the account of their host with that person's permission.  The registered patron shall be responsible for any lost or damaged materials.

Temporary residents must complete the registration form and provide a permanent address and name of their hometown library.  Temporary card holders will be limited to three items checked out at a time.  The Library Director may chose to allow temporary card holders to check out more items at a time on a case to case basis.
